  • 3.4 Deposits may be non-refundable once lab or surgical preparation begins.

Section 4 — User Conduct

You may not violate law, reverse engineer systems, scrape, impersonate, harass staff, record staff without consent where prohibited, or interfere with operations. Access may be terminated without notice.

Section 5 — Third-Party Services

Calendars, payment processors, lenders, messaging providers, and analytics platforms operate independently. We are not responsible for their acts, outages, or security incidents.

California Civil Code §1542 Waiver

California Civil Code §1542 waiver applies to third-party interactions. You waive the provisions of §1542, which states:

“A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party.”

You agree to defend and indemnify Fusion Dental Implants against claims arising from misuse, inaccurate medical history, non-compliance, or third-party reliance on your statements.

Section 10 — Force Majeure

We are not liable for delay or failure caused by laboratory delays, supply chain issues, illness, natural disaster, government action, or events beyond control.

Section 11 — Dispute Resolution

11.1 Mandatory Arbitration

All disputes must be resolved by binding AAA arbitration in Placer County, California.

11.2 Class Action Waiver

Claims must be brought individually.

11.3 Jury Trial Waiver

You waive jury trial rights.

11.4 One-Year Limitation Period

Any claim must be filed within one year of the event or is permanently barred.

11.5 Attorneys' Fees

Prevailing party entitled to fees where permitted by law.

Section 12 — Governing Law

California law exclusively governs regardless of residence.

Section 13 — Severability and Survival

If any clause is invalid, the rest remains enforceable. Liability, arbitration, and warranty provisions survive termination.

Schedule A — Treatment Warranty Addendum

(Zirconia Full-Arch Prosthetics)

This Schedule supersedes conflicting provisions but only for prosthetic warranty claims.

A1 — Scope

Covers zirconia full-arch prosthetic structure only. Excludes implants, screws, bone, tissue, surgery, anesthesia, and medical complications.

A2 — Lifetime Definition

Valid only for the original patient while continuous compliance requirements are met.

A3 — Eligibility

  • A3.1 Design Authority — All design decisions controlled exclusively by Fusion. Patient deviation voids warranty.
  • A3.2 Mandatory Maintenance — Cleaning every 6 months within 30-day grace period at Fusion or approved provider.

A4 — Exclusions

Bruxism, trauma, hygiene failure, smoking, drug misuse, systemic disease, unauthorized dental work, excessive force, or non-compliance void warranty.

A5 — Determination Authority

Fusion has sole discretion to approve or deny claims. Determination final.

A6 — Remedy

Repair or replace prosthetic only. No coverage for surgery, temporaries, travel, wages, or diagnostics. Replacement does not restart warranty.

A7 — Liability Cap

Maximum liability limited to amount paid for the prosthetic.

A8 — Modification Protection

Future policy changes do not retroactively affect compliant warranties.

A9 — Acceptance

Proceeding with treatment constitutes acceptance.


Schedule B — “Done with Dentures” Program Terms & Conditions

(Denture-to-Implant Upgrade Program)

This Schedule governs the “Done with Dentures” promotional program. All other provisions of the main Terms of Use remain in full effect.

B1 — Program Overview

The “Done with Dentures” program is designed for patients who currently wear removable dentures and wish to upgrade to a fixed, implant-supported solution. Qualifying patients may receive up to $2,500 off per arch toward their dental implant treatment.

B2 — Eligibility

  • Patient must currently wear removable dentures (full or partial) on the arch(es) being treated.
  • Patient must be a candidate for implant-supported restoration as determined by clinical examination and treatment planning by a Fusion Dental Implants provider.
  • Offer applies to new implant treatment cases only and cannot be applied retroactively to previously completed or in-progress treatment.
  • Patient must complete a consultation and receive a personalized treatment plan.

B3 — Discount Terms

  • The discount amount of up to $2,500 per arch is determined on a case-by-case basis and is subject to clinical evaluation, treatment complexity, and the specific treatment plan recommended.
  • The actual discount applied may be less than $2,500 per arch depending on individual case factors.
  • The discount is applied to the total cost of the implant-supported treatment and cannot be exchanged for cash, credit, or applied to other services.
  • Maximum combined discount for dual-arch (upper and lower) treatment is up to $5,000.

B4 — Restrictions

  • Cannot be combined with other promotional offers, discounts, or coupons unless explicitly stated in writing.
  • Offer is non-transferable and applies only to the patient who completes the consultation.
  • Fusion Dental Implants reserves the right to modify, suspend, or discontinue this program at any time without prior notice.
  • Offer has no cash value and is void where prohibited by law.

B5 — Clinical Determination

All treatment decisions, including eligibility for the program and the specific discount amount, are made at the sole discretion of the treating provider based on clinical findings. The advertised maximum discount is not guaranteed for every case. Final pricing is determined after comprehensive evaluation, imaging, and treatment planning.

B6 — Acceptance

By participating in the “Done with Dentures” program, the patient agrees to these terms and acknowledges that the discount is subject to clinical evaluation and case-specific factors.


Schedule C — New Patient Full-Arch Promotional Offer ($2,026 Off)

This Schedule governs the $2,026 off full-arch promotional offer for new patients. All other provisions of the main Terms of Use remain in full effect.

C1 — Offer Overview

New patients may qualify for up to $2,026 off full-arch dental implant treatment. For single-arch treatment, the discount is up to $1,013 per arch (half of the full-arch amount).

C2 — Eligibility

  • Patient must be a new patient of Fusion Dental Implants who has not previously received treatment at any Fusion Dental Implants location.
  • Patient must be a candidate for full-arch implant-supported restoration as determined by clinical examination and treatment planning by a Fusion Dental Implants provider.
  • Patient must complete a consultation and receive a personalized treatment plan.

C3 — Discount Terms
